Publishes a real price, then charges $200 to walk away
Compounded semaglutide from $116 a month is printed on the open site with no quiz in front of it — and Bliv's own refund policy deducts $200 for "clinical coordination and account setup" from the refunds it does grant, inside a window that shuts after 72 hours.

The verdict
Bliv does the thing most of this category refuses to do: it prints a number. Compounded semaglutide from $116 a month and tirzepatide from $199 sit on the public page, no intake required to see them, and the programme those numbers buy is spelled out in the company's own intake — provider review, the medication, injection supplies down to the alcohol swabs, free nurse consultations, free shipping in three to five days. The exit is where it turns. The refund policy gives you 72 hours, and only if nothing has been done and nothing has reached the pharmacy; after that there is no refund "regardless of service usage or medication dispensation" unless a doctor writes a letter. Even a refund you qualify for arrives $200 lighter, plus $24.99 for the provider consultation. That sits directly under a homepage promise of "no long-term commitments or cancellation fees", and the gap between those two sentences is the most useful thing on this page.

A good fit if
- You want to see a monthly figure before handing over your medical history
- You want the needles, syringes and swabs in the box rather than sourced separately
- You want a phone number — Bliv publishes one, and free nurse consultations are part of the programme
Look elsewhere if
- You are not certain you will stay past the first month — the refund window is 72 hours and closes hard
- You want to know which plan gets you the $116 before you buy: the tiers behind it are not published
- You want a named compounding pharmacy you can look up
- You live in Wyoming — it is the one state missing from Bliv's own licence list
What it actually costs
What they advertise
Compounded semaglutide "as low as $116 per month" and tirzepatide "as low as $199 per month", both footnoted "*based on plan structure and eligibility". A third card advertises "as low as $29 per month".
What you actually pay, month to month
$116 is a floor rather than a rate. Bliv publishes no plan tiers behind the phrase "plan structure", and the prices its own intake app resolves come from an authenticated API, so the figure a given reader is quoted is only visible after the questionnaire. What is fixed here is the exit cost, not the entry one.
The commitment trap
The $29 card is not a third price. It is CareCredit and Klarna financing spread over 24 months, and Bliv's refund policy states that where payment ran through CareCredit "the 15.99% service fee is non-refundable" — so the cheapest-looking number on the pricing grid is the one that adds a charge rather than removing one.
Money you will meet later

The cheapest headline number in this category is often the one with the least attached to it. Here is what Bliv bundles in, and what it does not.
Clinician consult
Included, and asynchronous by design
— a medical intake reviewed by a US-licensed provider, usually within a day, with no phone call and no video visit. The refund policy separately prices a provider consultation at $24.99, which is the only place a consult carries a number.
Bloodwork / labs
No lab requirement is published anywhere
— not on the site, not in the terms, not in the intake questionnaire.
Follow-ups and dose changes
"Continued access to medical support throughout your program"…
is listed as included, but the refill is your job: the terms make the patient responsible for requesting a refill once a month through a form.
Human support
Free nurse consultations are named in Bliv's own list of what…
the programme includes, and there is a published phone line — both rare in this category. Dietitian-led nutrition guidance exists, but as a community forum rather than a one-to-one service.
Injection supplies
Included: "Injection supplies (vial, syringes, alcohol swabs)"
The exception is brand Zepbound vials, where the intake tells you "You will need to purchase syringes when completing your order."

Free, in discreet packaging, typically 3-5 days after approval.
Pharmacy, labs and oversight
The pharmacy
Not named. The public site says a "U.S. pharmacy" and the intake says "FDA-regulated pharmacy in the U.S." — neither a 503A nor a 503B designation, and no facility a reader can look up. The intake is at least straight about the product itself, labelling compounded semaglutide doctor-trusted for weight loss and "(Not FDA approved)" in the same breath.

Compounded peptides are not FDA-approved products, and a 503A pharmacy compounds for a named patient while a 503B outsourcing facility manufactures in batches under CGMP. Neither is a quality guarantee on its own — a named facility is simply a fact you can check, and an unnamed “partner pharmacy network” is not. Where you can get it
Licensed in 49 states. Bliv's terms publish an actual state-licence list, which is more than most of this category manages — and it covers every state except Wyoming. Its own intake FAQ, one click away, claims Bliv is "Licensed to provide care in all 50 states", which its own list does not support.
The honest cons
The part they would rather you skipped
The homepage sells "No Contracts — total flexibility with no long-term commitments or cancellation fees", and the refund policy two clicks below it deducts $200 for "clinical coordination and account setup" before returning anything, adds $24.99 for the consultation, and shuts the refund window 72 hours after purchase whether or not you have used a thing. There is a second door too: if a policy change stops Bliv supplying a medication, the policy states that a credit is applied to your account and "No refunds will be issued" — the money stays with Bliv even when it is Bliv that cannot deliver. Any dispute over any of it goes to binding arbitration, under a clause asking you not to join a class action.

Read this before you buy
A provider comparison is half the decision. The other half is whether the compound does anything for you at all — and whether it is allowed where you compete. Growth-hormone secretagogues are banned in tested sport, year-round, and a prescription does not change that.
