The verdict
Willow prints its prices on the open page: semaglutide at $299 a month as a weekly injection or a daily sublingual tablet, tirzepatide at $399 in both formats, NAD+ at $199 — with the medical evaluation, physician access, 24/7 support and free 2-day shipping inside the number and no membership underneath it. Two things keep that from being the whole story. The $299 is a floor on an auto-renewing subscription — Willow's own FAQ says medication costs vary by dosage, and no dose-priced table exists anywhere — and the refund door closes the moment your prescription reaches the pharmacy. The other is in the terms, where Willow names its five partner pharmacies, a disclosure most of this category refuses to make: one of them is Empower Pharmacy in Houston, which drew an FDA warning letter in April 2025 over its sterile compounding conditions.

A good fit if

Look elsewhere if
- You have diabetes — Willow states it cannot accept you, though pre-diabetes and hypertension are welcome
- You live in one of the fourteen states missing from the published list
- You need to know your price at a higher maintenance dose before you start — that number is not published
- You want to choose the compounding pharmacy: routing the prescription to your own adds a platform fee, with the medication cost on you directly

What you actually pay, month to month
$299 a month for semaglutide and $399 for tirzepatide, billed as an auto-renewing monthly subscription. Treat both as floors: Willow's FAQ says medication costs vary by dosage and prescription, and no dose-tiered price table is published anywhere on the site.
The commitment trap
A floor on an auto-renewing program. The FAQ answers "Are there any additional fees?" with a flat no, then says two questions later that "medication costs vary by dosage and prescription" — so the bill at a higher dose is discovered inside the program, not before it. Cancellation requires two business days' notice ahead of the next processing date, and a full refund exists only if you cancel before the prescription is sent to the pharmacy.
Money you will meet later
- No membership fee and no separate consult charge — the evaluation, physician access and support ride inside the monthly price
- A platform fee if you send the prescription to a pharmacy of your own choosing — and you then cover the medication cost directly on top

Pharmacy, labs and oversight
The pharmacy
Named, but only at the bottom of the terms of service. The product pages say just "pharmacy-prepared"; the terms list five partner pharmacies with street addresses — Red Rock Home Pharmacy (Springville, UT), Epiq Pharmacy (Richardson, TX), Absolute Pharmacy (Lutz, FL), The Pharmacy Hub (Miami, FL) and Empower Pharmacy (Houston, TX). No 503A or 503B designation is given for any, and nothing tells a given patient which of the five will fill their order.
Oversight
Unlimited physician access is advertised, with the same doctor for continuity, and no per-visit fee published.
Compounded peptides are not FDA-approved products, and a 503A pharmacy compounds for a named patient while a 503B outsourcing facility manufactures in batches under CGMP. Neither is a quality guarantee on its own — a named facility is simply a fact you can check, and an unnamed “partner pharmacy network” is not. Where you can get it
Published — 36 states, listed in the FAQ. Alabama, Alaska, Arkansas, Delaware, Kansas, Louisiana, Mississippi, New Mexico, North Dakota, Rhode Island, South Dakota, Utah, Vermont and West Virginia are absent, and Washington, D.C. is not mentioned. Willow also does not accept patients with diabetes.
The honest cons
The part they would rather you skipped
One of the five partner pharmacies named in Willow's own terms — Empower Pharmacy, at 7601 N Sam Houston Pkwy W in Houston — received an FDA warning letter in April 2025, after inspectors found drug products intended to be sterile "prepared, packed, or held under insanitary conditions," rendering them adulterated; the same operating entity drew an earlier compounding letter in October 2021. Nothing on Willow's site mentions this, nothing tells a patient which of the five pharmacies will fill their order, and the way out — naming your own pharmacy — costs a platform fee plus the medication itself.

Read this before you buy
A provider comparison is half the decision. The other half is whether the compound does anything for you at all — and whether it is allowed where you compete. Growth-hormone secretagogues are banned in tested sport, year-round, and a prescription does not change that.
