Levers Health's real, binding limitation — Arizona only, stated in its own Terms of Service — is the single most important fact for most readers here, since it rules out nearly everyone outside that state regardless of how the rest of the site reads. Once you get past that, the site discloses more than our last check credited it for: a real address, a named and accredited pharmacy partner (Strive Compounding Pharmacy, LegitScript certified), and a genuine legal entity. The FAQ page's 404 is real, but the Terms of Service — where the state restriction and pharmacy name both live — loads fine at its own URL.
