Levers Health LLC's own Terms of Service state explicitly, in capital letters and confirmed live: "CURRENTLY LEVERS HEALTH LLC ONLY OPERATES IN ARIZONA AND SHIPS TO ARIZONA. WE CANNOT PROVIDE MEDICAL SERVICES OUTSIDE OF ARIZONA" — a hard, binding single-state restriction, a materially different finding than prior research's "state availability not disclosed" framing. Its named nationwide pharmacy partner, confirmed live in the same Terms document, is Strive Compounding Pharmacy, described as FDA-registered, 503A-compliant, and LegitScript certified — correcting prior research's claim that no LegitScript or PCAB accreditation existed; the certification applies to the pharmacy partner, not a separate Levers Health merchant badge. A physical address, 7067 East Dale Lane, Scottsdale, AZ, is confirmed live, another correction to prior research's "no physical address" finding. Its dedicated FAQ page does return a 404, confirmed live, matching prior research exactly, but its full Terms of Service loads without issue at a different, standard URL.

Levers Health's real, binding limitation — Arizona only, stated in its own Terms of Service — is the single most important fact for most readers of this review, since it rules out nearly everyone outside that state regardless of how the rest of the site reads. Once you get past that, the site discloses more than prior research credited it for: a real address, a named and accredited pharmacy partner (Strive Compounding Pharmacy, LegitScript certified), and a genuine legal entity. The FAQ page's 404 is real, but the Terms of Service — where the state restriction and pharmacy name both live — loads fine at its own URL.
