Foundayo vs Wegovy Pill: The Two GLP-1 Pills, Compared (2026)

Foundayo and the Wegovy pill both start at $149 a month self-pay. One is a peptide with strict morning rules; one is a small molecule you take any time.

Written by Derek OlssonSports Science Editor

The short answer: the Wegovy pill produced more weight loss on its label, and Foundayo is far easier to take. The Wegovy pill is oral semaglutide 25 mg, a peptide, and its label records an average 13.6% weight loss at 64 weeks against 2.4% on placebo5. Foundayo is orforglipron, a small molecule that is not a peptide at all, and its label records 11.1% at 72 weeks on the top dose against 2.1% on placebo4. The Wegovy pill must be taken on an empty stomach with a sip of water and a 30-minute wait; Foundayo can be taken with or without food, at any time of day45. Self-pay prices start at $149 a month for both and top out at $299910.

That is the whole decision in four sentences. The rest of this page is the detail behind each one, including the finding most comparisons skip: the two companies have each published a comparison that favors their own pill.

A Peptide Pill and a Non-Peptide Pill

The two drugs reach the same receptor by very different routes.

The Wegovy pill is semaglutide, the same peptide as the Wegovy pen. Peptides are normally digested before they can be absorbed, so the tablet carries an absorption enhancer called SNAC. It works locally in the stomach, briefly raising the pH and helping a small fraction of the dose cross the stomach lining14. That design is why the label is strict: food, drinks other than water, and other pills all compete with that narrow window. The label says to take the tablet in the morning on an empty stomach with no more than 4 ounces of water, then wait at least 30 minutes before eating, drinking or taking other oral medicines5.

Foundayo is orforglipron, described in its phase 3 paper as a small-molecule, non-peptide GLP-1 receptor agonist2. It survives digestion by chemistry rather than by a co-formulated helper, and its label says to take it once daily, with or without food4. The label's own pharmacology section reports no clinically relevant food effect on exposure4.

If you want the longer story of why most oral peptides fail and these two do not, see oral peptides for weight loss. If you came here looking for a tirzepatide tablet, none exists; tirzepatide pills explains what is sold under that name.

What the Trials Measured

Each drug has one large placebo-controlled weight trial behind its label, and the trials were not run side by side.

Wegovy pill (OASIS 4). 307 adults without diabetes were randomized 2:1 to oral semaglutide 25 mg or placebo. The published paper reports a mean change of −13.6% against −2.2% at week 641; the label reports −13.6% against −2.4%5. Novo Nordisk's approval announcement quotes a higher figure, 16.6%, which is the result among people who kept taking the tablet as directed rather than the result for everyone randomized8. Both numbers are real; they answer different questions.

Foundayo (ATTAIN-1). 3,127 adults without diabetes took orforglipron or placebo for 72 weeks. The paper reports −11.2% on the top dose against −2.1%2, and 18.4% of people on that dose lost at least a fifth of their body weight2. One thing will confuse anyone reading both documents: the paper names the doses 6, 12 and 36 mg, while the label states the same trial's strengths as 5.5, 9 and 17.2 mg4. It is the same trial and the same tablets, expressed two ways.

Comparing them directly is where it gets political. No head-to-head obesity trial exists. A Novo Nordisk–funded indirect comparison, which statistically adjusted OASIS 4 against ATTAIN-1, estimated about 3 percentage points more weight loss with the Wegovy pill and higher odds of stopping for side effects with orforglipron3. A Lilly-funded head-to-head trial in type 2 diabetes went the other way on blood sugar: orforglipron lowered HbA1c more than oral semaglutide at its 7 mg and 14 mg diabetes doses, though more people on orforglipron stopped for side effects (9–10% against 4–5%)12. That trial did not use the 25 mg weight-loss dose. Read each result with its sponsor in mind.

Food, Water and Daily Life

For many people this section decides it.

  • Wegovy pill: first thing in the morning, empty stomach, up to 4 ounces of plain water, then 30 minutes before coffee, breakfast or any other pill5. Miss the window and the dose is absorbed less well.
  • Foundayo: one tablet a day, whenever suits you, with or without a meal4. The label does add two practical cautions. Women using oral contraceptives are advised to switch to a non-oral method, or add a barrier method, for 30 days after starting and after each dose increase. And the top dose is capped at 9 mg for anyone also taking a strong CYP3A4 inhibitor4.

If you take several morning medicines, such as a thyroid tablet, the Wegovy pill's routine is a real cost, not a footnote. Ask the prescriber how they would sequence it.

How Each Dose Ladder Climbs

Both pills start low and step up monthly to keep nausea manageable, and the shape of each ladder matters because the price changes with the rung.

Wegovy pill: a fixed four-step climb. 1.5 mg for days 1 to 30, 4 mg for days 31 to 60, 9 mg for days 61 to 90, then 25 mg once daily from day 91 as the maintenance dose5. If 25 mg is not tolerated, the label suggests switching to the weekly injection at 1.7 mg rather than staying on a lower tablet. A missed tablet is simply skipped5.

Foundayo: three required steps, then optional ones. 0.8 mg for at least 30 days, 2.5 mg for at least 30 days, then 5.5 mg. From there the dose may go to 9 mg, 14.5 mg or 17.2 mg, each after at least 30 days, based on response and tolerability, with 17.2 mg as the ceiling4. In practice that means some people stop climbing at 5.5 mg, which on today's ladder is the $199 rung.

Never split, crush or double up to save money: both labels allow one tablet a day, and Foundayo's says to swallow it whole45.

Price Ladder by Dose

Both manufacturers sell their pill directly to self-pay patients, and their ladders are nearly identical. Prices below come from each manufacturer's own public pricing page, read in October 2026.

Wegovy pill through NovoCare Pharmacy: $149 a month for 1.5 mg, $199 for 4 mg, and $299 for both 9 mg and 25 mg, where a month is one 30-tablet bottle9. The page labels this a limited-time offer and says pricing will be updated after December 31, 20269.

Foundayo through LillyDirect: $149 a month for the 0.8 mg starting dose, $149 for 2.5 mg, $199 for 5.5 mg and $299 for 9 mg10. The 2.5 mg and 5.5 mg prices are marked as temporarily reduced through December 31, 2026. The 14.5 mg and 17.2 mg doses are $299 only when you refill within 45 days; miss the window and "regular prices" apply, which the page does not print10.

With insurance or Medicare: Lilly advertises as low as $25 a month for commercially insured patients whose plan covers Foundayo, with government beneficiaries excluded10. Both companies point eligible Medicare Part D patients to the Medicare GLP-1 Bridge program, which Lilly's page describes as $50 a month after prior authorization11, and NovoCare describes as $50 a month for Wegovy prescribed for weight loss9.

Several telehealth services prescribe one or both pills, and their headline figures do not always mean the same thing. LillyDirect's own Foundayo storefront still needs a prescription written elsewhere first. Ro quotes the drug price, then bills its membership on top. Amazon One Medical's "$25 a month" depends on both insurance and the savings card. Good Life Meds adds a monthly fee to its Wegovy pill headline, and NovoCare Pharmacy is the manufacturer's own dispensing route for the Wegovy pen and pill.

Side Effects and Stopping Rates

Both pills behave like every other GLP-1 drug: the stomach and gut take the brunt, mostly during dose increases.

On the Foundayo label, nausea affected 35% of people on 17.2 mg against 10% on placebo, vomiting 24% against 4%, and constipation 24% against 9%4. 10% on the top dose stopped permanently because of side effects, against 3% on placebo4.

On the Wegovy pill, gastrointestinal events were reported by 74.0% of people in OASIS 4 against 42.2% on placebo1. On the label, 6.9% stopped because of side effects against 5.9% on placebo5.

Different trials, different populations and different ways of counting, so treat the gap as a direction, not a precise margin. Both labels carry the same class boxed warning about thyroid C-cell tumors, and both drugs are contraindicated with a personal or family history of medullary thyroid cancer or MEN 245. The Wegovy pill is also labeled to reduce the risk of major cardiovascular events in people with established heart disease8; Foundayo's label is for weight management only4.

Who Should Still Pick an Injection

A pill is a convenience, not an upgrade. Injected semaglutide and injected tirzepatide carry the longest safety records and, for tirzepatide, larger average weight loss than either pill (20.9% at 72 weeks on the top dose in SURMOUNT-1)15, at a once-weekly rather than daily dose. Compounded injections also remain on sale at prices below the pills' top rungs. Compare them on our semaglutide provider board and the tirzepatide board, and see what the brand-name injection costs in semaglutide cost. FDA approval belongs to the brand products only; compounded versions are not FDA-approved13.

Bottom Line

Pick the Wegovy pill if you can keep a strict morning routine and want the larger label result and a cardiovascular indication. Pick Foundayo if you will only stay on a pill you can take at any time, with food, alongside other morning medicines. Starting prices are the same, $149 a month, and both top out at $299 self-pay as long as you meet each program's terms. Whichever you choose, make the decision with a prescriber who knows your other medicines. This page is educational, not medical advice.

Frequently asked questions

Which works better, Foundayo or the Wegovy pill?

On their own labels, the Wegovy pill recorded more average weight loss: 13.6% at 64 weeks against 11.1% at 72 weeks for Foundayo's top dose. The trials were run separately, though, and the only direct obesity comparison is an indirect statistical analysis funded by Novo Nordisk, which estimated about 3 percentage points in the Wegovy pill's favor.

Can I take Foundayo with food?

Yes. Foundayo's label says to take it once daily with or without food, and its pharmacology section reports no clinically relevant food effect. The Wegovy pill is the opposite: empty stomach, up to 4 ounces of water, and a 30-minute wait before eating, drinking or taking other pills.

How much does each pill cost without insurance?

Both start at $149 a month through the manufacturer. The Wegovy pill is $199 at 4 mg and $299 at 9 mg and 25 mg through NovoCare Pharmacy. Foundayo is $149 at 2.5 mg, $199 at 5.5 mg and $299 at 9 mg and above through LillyDirect, with the top two doses at $299 only when refilled within 45 days.

Is Foundayo a peptide?

No. Orforglipron is a small-molecule, non-peptide drug that activates the GLP-1 receptor. That is why it can be taken without food rules. The Wegovy pill is semaglutide, a peptide, which needs an absorption enhancer and an empty stomach to get through.

Do Medicare patients pay less?

Eligible Medicare Part D patients can use the Medicare GLP-1 Bridge program, which both manufacturers describe at $50 a month for their weight-loss products. Prior authorization and clinical criteria apply, and certain plan types are excluded.
