It does not clearly settle whether the $150 is incremental to a particular membership or how it relates to the final pharmacy bill. Obtain a complete quote rather than adding the figures or treating $150 as all-inclusive.

No separate evaluation purchase is required if you choose membership, because membership includes an evaluation. A standalone 30-minute evaluation is listed at $75 without a commitment. That visit can include assessment and ordering tests or medication, but GLP-1 prescriptions require an active membership and the products themselves cost extra.

Medication, labs and imaging are excluded from both the visit charge and membership fees; ordering them is included. Insurance or eligible self-pay discounts may help with separate charges. The site’s insurance FAQ and current insured membership choices differ, including Georgia and Texas options, so eligibility depends on the provider and plan. The approximate $335 basic PCOS lab panel is not a mandatory universal GLP-1 testing price.

The public area list, general FAQ and provider membership choices do not show the same coverage. A fixed state count would be misleading. Check the selected provider’s availability and your physical location for the visit; the FAQ says you need to be in a served state but do not have to reside there.

The practice describes a nurse-practitioner-led specialist team founded by Lynsey Johnson, DNP, APRN, FNP-C, and lists Strive and Striker as pharmacy partners. These are the practice’s disclosures, not independent confirmation of credentials, licenses or your individual pharmacy assignment. Ongoing monitoring and a fitness app are described, but the practice’s early chart-review results do not establish guaranteed outcomes.

Appointment cancellations or rescheduling require at least 48 hours’ notice to avoid the full charge. Monthly membership cancellation requires messaging the nurse practitioner through Optimantra at least 24 hours before the draft; the terms restrict refunds after the draft starts. Prepaid plans do not auto-renew. A $50 re-enrollment charge can apply after cancellation or expiration, subject to the stated seven-day grace provision; processed purchases and sent labs also have refund restrictions.
